Media/Photo Release

I give permission for my child to be photographed and/or recorded during the American Lutheran Church Youth Mission Trip to Salt Lake City, Utah (June 23–27, 2025). I understand that photos and videos may be used for church newsletters, social media, the church website, or other ministry-related materials.

Permission & Release

I give permission for my child to participate in the American Lutheran Church Youth Mission Trip to Salt Lake City, Utah from June 23 to June 27, 2025. I understand that they will be traveling with church staff and adult leaders, and participating in a variety of mission work, worship, and recreational activities.

In the event of illness or injury, I authorize the adult leaders to obtain medical treatment for my child if necessary. I understand that all reasonable efforts will be made to contact me before such action is taken.

I also release American Lutheran Church, its staff, and adult volunteers from any liability for injuries or incidents that may occur during the trip, except in cases of gross negligence or misconduct.
